To record and manage incidents and accidents that happen at Centrepoint Church or at a Centrepoint activity, so the person affected is cared for and the same thing is less likely to happen again.
2. Use this form for
- Accidents and injuries, on site or at any church activity, on-site or off.
- Near misses, where nobody was hurt but someone could have been.
- Medical emergencies, including where an ambulance was called.
- Property damage, church property or personal property.
- Behavioural disruption that needed a response and did not involve harm to a person.
3. Do not use this form for (use the Concern or Disclosure Report instead)
- Abuse, neglect or exploitation of any kind, disclosed or suspected.
- Self-harm, suicidal thoughts, or anything suggesting a risk to someone's personal safety.
- Concerning behaviour by a staff member, leader, volunteer or contractor.
- Domestic or family violence.
- Anything where you are unsure.
If an incident also involves harm to a person, fill in both forms.
4. Reporting obligations
If you are involved in, witness, or are told about an incident, respond promptly, calmly and supportively. Provide first aid or other assistance and keep the situation under control.
You must report whether you saw it yourself or were told about it by someone else, such as a parent, a participant or an external party. You do not need to be certain about what happened. Report what you saw or were told.
Submit this form as soon as you have addressed the incident.
5. Immediate response
Before the paperwork:
1. Attend to the person involved. First aid, ambulance, or pastoral care.
2. Remove or isolate any hazard so nobody else is affected.
3. Stop the activity if that is what it takes to bring things under control.
4. Tell the staff member or team leader on site.
6. Confidentiality
Treat everything on this form as confidential. Share it only with people who need it to respond.
7. What happens next
You will get a confirmation email with a copy of your responses. The Safer Churches Coordinator reviews every submission and where the incident is serious, it is reported to the church insurer and the Executive Team.
