Incident Report Form

1. Purpose
To provide a clear and consistent procedure for reporting and managing general incidents and accidents that occur within Centrepoint Church environments.
This form does not cover disclosures of abuse, which must be reported using the Disclosure Reporting Form.
2. Definitions
Incidents include, but are not limited to:
Accidents – When a person is injured or harmed in any way while on Centrepoint Church premises.
Near-misses – Events that had the potential to result in serious consequences or injury.
General incidents – Situations requiring emergency or pastoral response, such as self-harm, property damage, health emergencies, or behavioural disruptions that do not involve abuse.
3. Reporting Obligations
If a staff member, leader, or volunteer is involved in, witnesses, or becomes aware of an incident, they must respond promptly, calmly, and supportively—providing any necessary assistance (e.g., first aid) and maintaining situational control as appropriate.
Staff, leaders, and volunteers may also receive information from others (e.g., parents, participants, or external parties) about general incidents.
In all such cases, they are obligated to report the information immediately, regardless of whether it is based on direct knowledge or reasonable suspicion.
4. Management Procedures
Immediate Response
- Incidents must be reported immediately to a staff member or team leader at the event.
- The staff member or team leader should:
1. Attend to the person involved (e.g., provide first aid, call an ambulance, or offer pastoral care).
2. Remove or isolate any known hazards from others.
3. Discontinue the event or activity if necessary to bring the situation under control.
Documentation
- All relevant details should be recorded on this Incident Report Form, including observations or information leading to knowledge or suspicion of the incident.
- The form may be completed by a staff member, leader, or volunteer.
Confidentiality
- All staff, leaders, and volunteers must treat incident-related information as confidential.
Escalation
- If a Pastor is informed of the incident, they are responsible for ensuring safety and escalating the report appropriately via the Incident Management Escalation Table.-
